Oral cancer, also referred to as mouth cancer, is a disease where malignant cells develop in the tissues of the mouth or on the lips. It belongs to a group of cancers known as head and neck cancers and most frequently begins in the squamous cells that line the surfaces of the tongue, mouth, and throat. DNA acts as the instructional manual for every cell, determining when it should grow, divide, and expire. When these instructions are damaged through mutations, the “off switch” for cell division fails to function. Consequently, the cells continue to multiply rapidly and uncontrollably, accumulating into a mass known as a tumour. Over time, these abnormal cells can outcompete healthy cells for nutrients and space, eventually leading to the destruction of the surrounding oral structures. 

The Role of Squamous Cells 

Most oral cancers begin in squamous cells, which are the thin, flat cells that make up the lining of the mouth and throat. When cancer originates in these cells, it is clinically termed squamous cell carcinoma. Under normal conditions, squamous cells are constantly replaced as they wear out from the friction of eating and speaking. However, if the DNA in the regenerative layer of these cells is damaged, the replacement process becomes flawed. Instead of creating healthy new lining, the body produces layers of malignant squamous cells that do not shed or die off as they should. 

Precancerous Conditions and Dysplasia 

In many instances, oral cancer does not start suddenly but is preceded by visible changes in the mouth lining known as dysplasia. These are precancerous states where cells appear abnormal under a microscope but have not yet become fully cancerous or invasive. These changes often present as leucoplakia, which are white patches, or erythroplakia, which are red, velvety patches on the soft tissues of the mouth. The National Institute for Health and Care Excellence (NICE) provides clinical guidelines for the recognition and referral of suspected oral cancers to ensure timely assessment. Monitoring these lesions is critical because they indicate that the cellular environment has become unstable. 

Factors Triggering the Beginning of Cancer 

The initial mutations that cause oral cancer to begin are often the result of cumulative damage from external irritants. Carcinogens found in tobacco products and alcohol can chemically alter the DNA of oral cells, especially when used frequently over long periods. Furthermore, certain strains of the Human Papillomavirus (HPV) are known to integrate their own genetic material into the DNA of the host’s oral cells, which can trigger the cancerous process. These factors do not cause cancer instantly; rather, they create a persistent environment of cellular stress that increases the probability of a permanent genetic error occurring. 

Invasion and Progression 

Once the cancerous process has begun and a tumour has formed on the surface of the oral lining, the disease can progress by invading deeper layers of tissue. This is known as invasive carcinoma, where the malignant cells break through the basement membrane that separates the surface lining from the underlying connective tissue, muscle, and bone. Once the cancer reaches this depth, it gains access to blood vessels and the lymphatic system. This allows individual cancer cells to break away from the primary site and travel to other parts of the body, most commonly the lymph nodes in the neck.
