Can Adults Take Autism ADOS‑2 Only?

Author: Lucia Alvarez, MSc | Reviewed by: Dr. Rebecca Fernandez, MBBS

No, using the ADOS-2 only for adults is not generally recommended for a complete and accurate autism diagnosis. While the ADOS-2 (Autism Diagnostic Observation Schedule, Second Edition) is a valuable tool, it was never designed to function as a standalone assessment. Relying on it alone can overlook critical information needed for an accurate clinical picture.

Why ADOS-2 Alone Isn’t Enough for Adult Diagnosis

Here’s why combining tools is essential when assessing adults:

Standalone Assessment

The ADOS-2 evaluates observable behaviours like social interaction, communication, and restricted interests. However, as a standalone assessment, it doesn’t account for developmental history or internal experiences, both of which are essential to understanding autism in adults, especially those who mask or compensate in social settings.

Diagnosis Method

A comprehensive diagnosis method includes structured interviews (like the ADI-R), self-report measures, clinical observation, and input from family or long-term contacts. Using the ADOS-2 only for adults might miss subtle but clinically significant traits that don’t present clearly in structured settings.

Tool Sufficiency

Although the ADOS-2 is highly regarded, its tool sufficiency depends on how it’s used. In isolation, it may lead to underdiagnosis or misinterpretation. That’s why clinical judgement and supporting assessments are critical to ensure a valid result.
