Is Autism ADI‑R Undergoing Revision?

Author: Lucia Alvarez, MSc | Reviewed by: Dr. Rebecca Fernandez, MBBS

No, an autism ADI-R revisions have not introduced a new edition since its original version, but several tool improvements have been proposed to bring updated questions and enhanced structure in line with evolving diagnostic standards. While the core content, such as the 93-item semi‑structured interview, remains unchanged, researchers continue to explore ways to refine algorithms, shorten protocols, and adapt for broader ASD subtypes.

What Is Being Updated or Considered?

Here’s a look at current trends in ADI‑R revisions and enhancements:

Algorithm Enhancements

Work stemming from DSM‑5 criteria has led to updated scoring approaches and refined algorithms, particularly for younger children. These aim to maintain sensitivity and specificity while reflecting modern diagnostic frameworks.

Tool Efficiency Research

A study using machine learning examined over 1,600 ADI‑R interviews and identified a subset of just five items, relating to stereotyped language, social verbalisation, reciprocal conversation, gaze and sensory responses, capable of maintaining around 95 percent accuracy compared with the full ADI‑R.

Protocol Streamlining

Recent practice improvements include consolidated algorithm forms and digital scoring aids, making administration smoother and less prone to human error, without altering the content of the tool itself.

What To Expect Next

Although a new edition has not yet been released, ongoing research is shaping the future of the ADI‑R. Anticipated developments include:

  • Though a full ADI‑R version update remains absent, ongoing research suggests future possibilities such as abbreviated versions or revised domains.
  • Any future ADI‑R release would likely include updated questions, enhanced structure and expanded validity across diverse groups.
  • Until then, the existing version continues to be widely respected and remains the standard due to its extensive validation and reliability under expert clinical use.

In short, while no official edition replacement is currently underway, small but significant ADI-R revisions and tool improvements are actively being developed to enhance interview efficiency and diagnostic relevance.
