Does the NHS ADHD trans children test for ADHD in transgender kids? 

As of mid-2025 in UK healthcare, England, there are plans for the NHS to introduce screening for neurodevelopmental conditions, including NHS for ADHD in trans children referred to gender identity services. This comes from the Cass Review and draft guidance aimed at more holistic assessments of young people under the NHS. 

What the recent guidance says and what it doesn’t 

Here’s a brief overview what recent NHS for ADHD in trans children comprises of; 

  • The Cass Review, published in April 2024, recommended that children referred to NHS Children and Young People’s Gender Services undergo assessments that cover neurodevelopmental conditions, child psychiatry, mental health, family relationships, and social context. 
  • Under proposed, but not yet fully implemented, revised specifications, screening for ADHD, autism, and other learning or neurodevelopmental differences will become more routine in gender clinics. 
  • However, “screening” does not necessarily mean automatic diagnostic assessment or immediate access to ADHD treatment.  

Key challenges and current gaps

  • Access to ADHD assessments in the NHS already faces long waiting lists, delays, and resource constraints, regardless of gender identity. 
  • There is concern among some stakeholders that making ADHD screening mandatory for trans children before gender-affirming care could lead to additional gatekeeping or unintended delays. Critics argue this could pathologize identity rather than provide support. 
  • The draft guidance is still going through consultation, and full implementation and how it works in practice are not yet confirmed. 

In conclusion, the NHS is moving toward more consistent ADHD screening for trans children, but the policy is still being finalized and there are important details and protections to watch out for.
